    Job Description
    The Community Health and Wellness Coordinator assures quality of care and accountability for community and public health services offered by Stoughton Health. This position designs and markets Stoughton Health’s community health services to the general public and Stoughton Health constituant groups. The Community Health and Wellness Coordinator will thrive on working autonomously and be results driven. This position will be self-motivated and display excellent technical and interpersonal skills necessary to improve community health.

    This position is responsible for compliance with health standards as defined by State and Federal government and professional standards. Duties include promotion of Stoughton Health’s community and public health services as well as conducting screening examinations and initiating necessary education related to health and safety. The Community Health and Wellness Coordinator assures confidential communication and quality of care to area residents. Education related to health and safety prevention is provided by this role.

    Under the direction of the Vice-President of Growth and Development the Community Health and Wellness Coordinator initiates services to benefit the community and Hospital alike.

    Qualifications include: Bachelor's degree in Nursing, Exercise Physiologist, or related degree in Health & Wellness. Prefer someone with a Master's degree. Looking for someone with at least three years of experience as a health professional. Experience in public health and marketing is strongly preferred. Knowledge in patient care practice, population health, wellness and adult learning principles is also strongly preferred. Must be licensed as a Registered Nurse or Exercise Physiologist in good standing in the State of WI. Must also have current BLS certification.

    *Full-time, Days, M-F, 8 a.m. - 4:30 p.m., 32-40 hours/week, 64-80 hours bi-weekly, no weekends/holidays.
