• Pharmacist

    Stoughton Health has an excellent opportunity for a Pharmacist to join our Pharmacy team on a Full-Time basis.
    The Pharmacist is responsible for preparing, dispensing, and managing medications in a hospital setting, ensuring safe and effective use of pharmaceutical drugs for inpatient and outpatient care across all age categories.  The Pharmacist works collaboratively with medical providers, nurses, and other healthcare professionals to optimize patient outcomes. The Pharmacist acts independently within the scope of their license and areas of expertise with the help of general policies, procedures, and practice guidelines and support from the Pharmacy Manager. The Pharmacist ensures that patient care is maintained through compliance with the department and institution policies, rules, and regulations promulgated by the Board of Pharmacy, and other applicable regulatory bodies.

     

    Essential Functions:

    • Complete accurate order review, preparation, and dispense of medications
    • Perform medication therapy monitoring to ensure appropriate medication regimens
    • Complete admission and discharge medication reconciliation
    • Collaborate with medical providers on the selection, dosage, interactions, and side effects of medication
    • Ensure compliance with hospital policies, legal, and regulatory standards related to the practice of pharmacy
    • Provide direct supervision of pharmacy technician, intern, and student daily activities

     


    Position Requirements:

    • Required: Bachelor’s Degree in Pharmacy
    • Preferred: Doctorate in Pharmacy
    • Licensure: Registered as a Pharmacist with the State of Wisconsin Department of Safety and Professional Services
    • Current BLS certification at hire and ACLS within 6 months of hire.
    • Computer Competency: Basic Computer Skills are expected for this position. Access email, reference internal websites, access electronic health records, and use online learning tools to remain current with continuing education requirements.
    • Microsoft Office Suite Skills: Beginning Skills – Able to perform basic things such as opening a document, change font, save a document, write a letter.

    Work Shift:

    • Full-Time, 1.0 FTE
    • 40 hours per week
    • Scheduled Days: Variable days
    • Pharmacy Hours:
      •  Monday – Friday 7:00 am – 6:00 pm
      • Weekends – 7:00 am – 2:30 pm
    • Every 3rd Weekend, 7:00 am – 2:30 pm
    • 2 Holidays per year (rotating), 7:00 am – 2:30 pm
